Prior to participation, each player needs the required documents on file.

-Copy of a birth certificate
-Completed School enrollment form (For those who are home schooled, three forms verifying residency within league boundaries can be used)
-Completed Medical release form

A volunteer form is required each season, for coaches and any volunteer who comes in regular contact with the kids.  Even for those who do not wish to coach, we suggest filling out and submitting the volunteer form, as there are numerous volunteering opportunities.
